New Patient Checklist

Welcome to our practice! To provide you with the highest level of care and to ensure your first visit is focused on your health rather than paperwork, we require the following information before we can finalize your appointment.

Please have the following items ready when you call to book:

1. Insurance & Billing Information

  • Primary Insurance Card: We need the Member ID, Group Number, and the "Provider Services" phone number on the back of the card.
  • Secondary Insurance: If you have Medicare Supplemental or a secondary plan, we must have this on file to coordinate your benefits.
  • Referral Status: If your insurance (HMO/POS) requires a referral from your Primary Care Physician (PCP), please ensure they have sent it to our office via Fax@512-547-5779 before calling.

2. Clinical Records

Dr. Rupakula cannot provide a comprehensive consultation without reviewing your history.

Please ensure we have received:

  • Recent Labs: Any blood work from the last 6 months (specifically A1c, Thyroid panels, or Metabolic panels).
  • Imaging Reports: If you are being seen for thyroid nodules or pituitary issues, we need the written reports for your Ultrasounds, CT scans, or MRIs.
  • Biopsy Results: If you have had a prior fine-needle aspiration (FNA) of the thyroid gland,we require the pathology report.

3. Diabetes Technology (If Applicable)

  • If you use a Continuous Glucose Monitor (CGM) or an Insulin Pump, please Bring your devices. Please bring your pump, receiver, and any specialized cables to your appointment.

4. Medication List

  • Please have a list of all current medications, including dosages and how often you take them. This includes any over-the-counter supplements or vitamins.

5. Appointment Arrival Time

  • Please arrive 15 minutes prior to your scheduled time for vitals and data syncing.

IMPORTANT PRACTICE POLICIES

  • The 24-Hour Hold
  • If you do not have your insurance information at the time of your call, we can place a "soft hold" on an appointment slot for 24 hours. If the information is not provided within that window, the slot will be released to another patient.

Cancellation Policy
